PT-141 (Bremelanotide)

PT-141 (Bremelanotide) is a melanocortin receptor agonist derived from Melanotan II, but specifically designed for sexual dysfunction. Unlike PDE5 inhibitors (Viagra, Cialis) that work on blood flow, PT-141 works centrally in the brain by activating MC3 and MC4 receptors involved in sexual arousal. It is effective for both men and women, and is FDA-approved as Vyleesi for hypoactive sexual desire disorder in premenopausal women.

Frequency

As needed, no more than once per 24 hours, max 8x per month.

Dosing Protocol

500 mcg to 1 mg per use (10-20 units), as needed 2 hours before sexual activity. For 5 mg vial reconstituted with 1 mL, 0.05 mg per unit — 10 units = 500 mcg. For 10 mg vial reconstituted with 2 mL, 0.05 mg per unit — 10 units = 500 mcg, 20 units = 1 mg. Do not exceed 20 units. Consult with doctor.

Reconstitution

Add 1 mL bacteriostatic water to a 5 mg vial, or 2 mL to a 10 mg vial.

Timing

2 hours before sexual activity.

Potential Side Effects

  • Nausea (most common)
  • Facial flushing
  • Headache
  • Slight blood pressure increase

Important Warnings

  • FDA-approved only for premenopausal women with HSDD (as Vyleesi)
  • Do not use more than once in 24 hours or more than 8 times per month
  • May cause transient blood pressure increase
  • Not for use during pregnancy
